Care Coordination/Clinical Integration Specialist (CIS)

Position Duties:
  The ED CIS reviews and screens ED inpatient admissions and observations as specified by the facility’s Utilization Management/Review Committee for documentation completeness and compliance with patient status. The ED CIS facilitates accurate documentation for severity of illness and medical necessity. The ED CIS interacts with physicians, case managers, and nursing staff and provides guidance and recommendations for admission or observation disposition. The CIS assesses patients for present-on-admission (POA) conditions to ensure accurate documentation regarding hospital acquired conditions (HAC). The CIS communicates the appropriate concurrent information to the inpatient Case Managers and Clinical Documentation Specialists (CDS).
Educational Requirements:
  •ASN Degree required, BSN strongly preferred.
•Current CT Registered Nurse license
Minimum Requirements:
  •4 years of acute hospital experience required, must have recent experience within the past 2 years in a ICU, CCU, ED or strong Med/Surg
•Knowledge and understanding of Medicare Part A, including Observation and Inpatient medical necessity.
•Knowledge of regulatory environment, including POA/HAC and core measures
Preferred Qualifications:
  •CCM certification, strongly preferred
•Organizational, analytical, writing and interpersonal skills
•Dependable, self-directed
•Critical thinking, problem solving and deductive reasoning skills
•Case management experience preferred
•Strong computer skills
•Understand and support CDMP® documentation strategies, preferred
•Familiar with Medicare Part B
Hours:
  40 hours, 11:00am - 11:00pm (3 - 12 hour shifts per week and every other Saturday, 8 hour shift)
